■国立大学病院“過去最大”赤字で医療崩壊の危機

 

400億円の赤字見込み

 2025年10月3日、『国立大学病院会議』は会見で、国立大学病院の赤字額が『2023年度=60億円、2024年度=286億円、2025年度(見込み)=400億円超』と発表し、大鳥精司会長は「過去最大の危機を迎えると言っても過言ではない」としています。

 

施設・設備費を減らすしかなく…

 赤字の要因は支出増加で、『医療費=258億円増、光熱・水道費=9億円増、人件費=325億円増、業務委託費など=89億円増』(前年度比)となっています。そんな中、76億円減っているのが『施設・設備費』で、大鳥会長によると「新たな医療機器が買えない。98%の医療機器等が更新できない状況」ということです。

 

検査できない状況が起こる恐れ

 患者への影響について問われた東京科学大学病院・藤井靖久病院長は、「人工呼吸器やECMO(エクモ/人工心肺装置)は更新している。ただ、画像診断装置・CTなどは直接患者に影響がないので、長く使う。患者の検査ができないという状況がいつ起こってもおかしくない」と答えました。

Q.都心部もですが、地方ではすでに医療崩壊が起こっていますよね? (日本医師会・松本吉郎会長) 「完全に医療崩壊が起こっています。経営努力はしていますが、物価高騰や人件費の上昇で経営が立ち行かなくなり、支出・経費が膨らんで、どこも大幅な減益になっています」

■National University Hospitals Face Crisis of Medical Collapse Amid Record-High Deficits

 

 

Projected Deficit of 40 Billion Yen

 

On October 3, 2025, the National University Hospital Council announced at a press conference that the deficit for national university hospitals would be ‘FY2023 = 6 billion yen, FY2024 = 28.6 billion yen, FY2025 (projected) = over 40 billion yen’. Chairman Seiji Otori stated, “It is no exaggeration to say we are facing the greatest crisis ever.”

 

 

 

Facility and Equipment Costs Must Be Cut...

 

The deficit stems from increased expenditures: ‘Medical expenses = ¥25.8 billion increase, Utilities = ¥0.9 billion increase, Personnel costs = ¥32.5 billion increase, Outsourcing fees, etc. = ¥8.9 billion increase’ (compared to the previous fiscal year). Amidst this, ‘Facility and equipment costs’ have decreased by ¥7.6 billion. According to Chairman Otori, "We cannot purchase new medical equipment. We are in a situation where 98% of medical equipment cannot be replaced."

 

 

 

Risk of situations where tests cannot be performed

 

When asked about the impact on patients, Dr. Yasuhisa Fujii, Director of Tokyo University of Science Hospital, responded, “We are replacing ventilators and ECMO (extracorporeal membrane oxygenation) devices. However, imaging diagnostic equipment like CT scanners doesn't directly affect patients, so we use them for longer. It wouldn't be surprising if situations where patients cannot be tested arise at any time.”

Q. While urban areas are affected, rural regions are already experiencing a collapse of medical services, correct? (Dr. Yoshiro Matsumoto, President of the Japan Medical Association) “A complete collapse of medical services has occurred. Despite management efforts, soaring prices and rising labor costs have made operations unsustainable. Expenses and costs have ballooned, leading to significant profit declines everywhere.”
